SCHOOL REGISTRATION PROCEDURES
All 7th and 8th grade students living on US Marine Bases on Okinawa will attend Lester Middle School. Registration is a simple process.
When you arrive on island, please come to the Main Office of the school. Please bring the following items with you, as they are required for enrollment:
- Sponsor's Orders or Verification of Employment
- Area Clearance or Dependant Entry Approval (for USMC & Navy only)
- Copy of Student's Passport
- Copy of Immunization Card
- Social Security Card or ID Card showing Student's Social Security Number
- eMail Work/Home Address
At school, the registrar will have you fill out the following forms:
- DSF 100 or DSPA 2030.2
- SD form 600
- Space Available Notification (If applicable)
- DSPA form 120.1, Health Record
- DSPA form 122.1, Immunization Verification (from immunization clinic)
- Special Needs Form
- Special Education Form
- Publication Permission/Field Trip Permission form
- Race/Ethnic Identification Questionnaire
